Moderate Stage Skilled Nursing Community

Our dementia care team guides and supports you, and your family member as they transition to a new stage-specific level of care. Continuous assessment by the staff ensures that residents move to a level of care that is more appropriate to their changing cognitive, physical, and therapeutic needs. Every move is a positive experience that brings the resident more comfort, more satisfaction, and less anxiety.

Mild-to-Moderate-Stage Skilled Nursing Care

Two residential communities on the first first floor are designed for mild-to-moderate-stage dementia. The environment is home-like, and we encourage families to decorate rooms with pictures of family, friends, trips, and mementos from special times in their family member's life.

At the Mild to Moderate Stage we help residents to:

Participate in physical self-care;
Maintain physical strength;
Focus on preserved cognitive capacity;
Focus on personal preferences; and
Receive meaningful cognitive stimulation
Maintain a reassuring structure and daily routine;
Communicate verbally on a regular basis;
Eat with limited assistance; and,
Participate in the community’s programs and daily life.
